<sec><title>Purpose</title>
<p> The purpose of this study was to validate the newly proposed periodontal disease index &quot;PerioQuotient (PQ)&quot; based on a periodontal disease symptoms questionnaire developed by brainstorming and the Delphi method by the 32nd Executive Committee of the Korean Academy of Periodontology.</p></sec>
<sec><title>Materials and Methods</title>
<p> A total of 135 subjects were recruited. Age, gender, and clinical indices of periodontal disease were measured. Periodontitis stage was determined, and an 18-question questionnaire was administered. The internal consistency and validity of the questionnaire were examined using Cronbach&#x00027;s alpha and principal component analysis. Receiver operating characteristic (ROC) analysis, area under the curve (AUC), sensitivity, and specificity were investigated to assess the diagnostic test performance of the questionnaire.</p></sec>
<sec><title>Results</title>
<p> The mean age of the 135 subjects was 48.8 &#x000b1; 17.1 years (male 77 (57.0%), female 58 (43.0%)). The average response to the 18 questions was 7.4 &#x000b1; 3.1. Reliability testing of the PQ-18 showed that the Cronbach&#x00027;s alpha was 0.68. The original PQ-18 had an AUC of 0.806 (95% CI: 0.709-0.903). Sensitivity was 0.76, and specificity was 0.77. Applying weighting factors according to Firth&#x02019;s logistic regression analysis to the answers increased the AUC to 0.964 (95% CI: 0.934-0.993). Sensitivity was 0.893, and specificity of 1.0 was achieved.</p></sec>
<sec><title>Conclusion</title>
<p> The PQ-17, excluding Question 15 (&quot;No scaling for two years&quot;), was shown a valid and reliable tool for the diagnosis of periodontitis. The PQ index presented in this study achieved high sensitivity, specificity, and AUC, suggesting its potential use as a diagnostic tool for periodontal disease status assessment.</p></sec>

<title>서론</title>
<p>세계보건기구는 전세계적으로 약 35억명, 약 40% 정도의 인구가 하나 이상의 구강질환을 겪고 있다고 추정하며, 당뇨병, 심혈관질환, 만성호흡기질환 등의 비전염성 만성질환을 합한 것보다 10억건 더 많은 것으로 보고했다&#x0005b;<xref ref-type="bibr" rid="b1-jkda-2026-64-6-001">1</xref>&#x0005d;. 치주질환은 대표적인 구강질환 중 하나로 건강보험심사평가원의 2023년 자료에 의하면 &#x02018;외래 다빈도 상병별 통계&#x02019;에서도 1위를 차지하고 있다&#x0005b;<xref ref-type="bibr" rid="b2-jkda-2026-64-6-001">2</xref>&#x0005d;.</p>
<p>치주염은 다양한 요인에 의해서 발생하는 질환으로 미생물에 의해 발생하며 숙주의 면역반응으로 매개되는 염증으로&#x0005b;<xref ref-type="bibr" rid="b1-jkda-2026-64-6-001">1</xref>&#x0005d;, 치아의 부착기구의 파괴로 치주낭, 치은출혈, 치조골 흡수 등이 나타나며 좀 더 진행이 되면 치아의 상실로 이어질 수 있는 질환이다&#x0005b;<xref ref-type="bibr" rid="b3-jkda-2026-64-6-001">3</xref>-<xref ref-type="bibr" rid="b5-jkda-2026-64-6-001">5</xref>&#x0005d;. 최근에는 치주질환과 다양한 비전염성 만성질환과의 관련성에 대해서도 보고되고 있다&#x0005b;<xref ref-type="bibr" rid="b6-jkda-2026-64-6-001">6</xref>,<xref ref-type="bibr" rid="b7-jkda-2026-64-6-001">7</xref>&#x0005d;. 따라서 조기에 치주질환을 진단하여 치료를 하게 된다면 치아상실로 인한 사회, 경제적 손실을 줄일 수 있다. 그러나 치주질환 중 초기 치은염의 경우에는 치은 부종이나 출혈 등의 증상이 가역적으로 해소되는 경우가 있기 때문에 치은출혈, 구취 및 부종 등의 자각증상이 있어도 치주치료로 연계시키지 못하는 환자들이 많다&#x0005b;<xref ref-type="bibr" rid="b8-jkda-2026-64-6-001">8</xref>&#x0005d;. 최근 노인 인구의 증가에 따라 치과를 방문하지 못하는 노인 환자들의 치주치료 필요도 평가를 위한 지수 개발에 대한 필요성도 증가하고 있다&#x0005b;<xref ref-type="bibr" rid="b9-jkda-2026-64-6-001">9</xref>,<xref ref-type="bibr" rid="b10-jkda-2026-64-6-001">10</xref>&#x0005d;. 또한 &#x02018;구강전신건강 통합프로그램 모형 개발 연구&#x02019;와 &#x02018;구강-전신건강 통합관리 보건소 모형 개발 연구&#x02019;에서도 환자의 구강건강 상태를 평가할 수 있는 지수 개발이 필요함을 강조하고 있다&#x0005b;<xref ref-type="bibr" rid="b11-jkda-2026-64-6-001">11</xref>,<xref ref-type="bibr" rid="b12-jkda-2026-64-6-001">12</xref>&#x0005d;.</p>
<p>대한치주과학회는 2009년부터 시행해 온 &#x0201c;잇몸의 날&#x0201d; 대국민 홍보사업의 일환으로 환자들의 잇몸 상태를 평가하기 위해서 치주건강과 관련된 질문들을 모아 2013년 치주건강지수(Perio-Quotient index, PQ지수)를 발표했다. 국내에서 환자들의 주관적인 증상을 이용하여 질환 정도를 객관화하려는 첫 시도로, 치주전문의들이 개발한 12개의 문항을 통해 일반인도 잇몸 건강 상태를 평가할 수 있는 지수라는데 의의가 있다. 치주건강지수는 숙련된 치주전문의들의 임상 경험을 토대로 개발하였으나, 실제 치주질환 정도와의 상관관계를 객관적으로 분석한 데이터는 부족하다는 한계가 있다. 최근 치주질환 환자들을 대상으로 한 임상 연구에 따르면 환자 스스로가 평가한 치주상태와 실제 임상검사 결과에는 차이가 있다는 보고가 있다&#x0005b;<xref ref-type="bibr" rid="b13-jkda-2026-64-6-001">13</xref>&#x0005d;.</p>
<p>이번 연구를 통하여 환자의 치주질환 자각증상을 기반으로 하는 18개의 문항과 환자의 임상적, 방사선학적 치주상태와의 상관관계를 비교 분석하고, 가장 유의미한 문항을 선정하여 환자의 치주상태를 평가하는 객관화된 지수로 개발하고자 하였다.</p>

<sec>
<title>대상 및 방법</title>
<p>본 연구는 각 참여 기관의 기관윤리위원회의 승인을 득한 후 진행되었다(승인번호: DKUDH IRB 2024-10-005, KC-23QIDI0893, HYUH2024-08-028-001).</p>
<p>대한치주과학회 치주과 전문의 40명은 브레인스토밍과 델파이 기법으로 18개의 문항을 선정하여 치주질환 설문지를 작성하였다(<xref rid="f1-jkda-2026-64-6-001" ref-type="fig">Fig. 1</xref>).</p>
<p>2024년 9월에서 2025년 6월 사이 서울성모병원 치과병원, 한양대학교병원 치과, 단국대학교 치과병원을 방문한 성인 중 자발적으로 동의서에 서명한 환자를 대상자로 하였다. 연구는 Declaration of Helsinki에 입각하여 진행되었으며, 피험자는 연구의 목적과 연구 참여 중 일어날 수 있는 정신적, 신체적 위해를 충분히 이해한 후 동의서에 서명하였다. 선정기준은 파노라마방사선사진 촬영 결과 (1년 이내) 및 치주낭 탐침 검사 결과의 연구목적 제공 및 설문지 작성에 동의한 대상자로 잔존치아가 없는 무치악 환자 및 급성 감염병이 있는 환자 및 응급한 치료가 요구되는 환자가 제외되었다.</p>
<p>동의서를 작성한 연구 참여자의 파노라마방사선사진을 촬영한 후 구강 검사 및 치주낭 탐침 검사를 시행하고 설문지를 제시하여 답변을 받았다(<xref rid="f1-jkda-2026-64-6-001" ref-type="fig">Fig. 1</xref>). 파노라마방사선사진 및 치주낭 탐침검사 결과와 설문지의 답변을 종합하여 분석하였다.</p>
<p>연구대상자 수는 신뢰수준 95%, 예상반응율 0.5, 오차범위 &#x000b1; 5%를 가정하여 최소 385명이 필요하고, 설문지 답변이 부족한 대상자의 탈락률 5%를 고려하여 405명이 필요하나, 예비 연구로서 그 1/3인 135명에서 연구가 시행되었다. 가톨릭대학교 서울성모병원에서 50명, 단국대학교치과병원에서 50명, 한양대학교병원에서 35명 등록하여 시행하였다.</p>
<p>인구사회학적 특징으로 연령, 성별을 조사하였고, 파노라마방사선사진 상에 나타나는 잔존 치아 수, 임플란트 수, 치조골 소실 정도(marginal bone loss, MBL), 치아 우식, 이환 치아 수, 치주낭 탐침 검사 시 치주낭 탐침 깊이(probing depth, PD), 치은퇴축(gingival recession, GR), 임상부착수준(clinical attachment level, CAL), 전악 치태 침착 여부(full mouth plaque score, PI), 탐침 시 출혈 여부(bleeding on probing, BOP), 및 그 외 치과질환과 파노라마방사선사진 상의 인접면 골 소실 기준으로 2017 World Workshop Classification3)에 따른 치주질환의 진단기(stage)를 기록하였다. 치주질환의 기준을 치주염 2기 이상 또는 치주염 1기 시 치주낭 깊이가 4mm 이상이면서 탐침 시 출혈이 있는 치아가 1개 이상으로 설정하였다.</p>
<p>설문 문항의 내적 일관성 신뢰도는 크론바흐 알파(Cronbach&#x00027;s alpha)로, 타당도 검증은 주성분 분석(principal component analysis)으로 확인하였다.</p>
<p>설문 문항 세트의 예측 정확도를 평가하기 위하여 수신기 조작 특성(receiver operating characteristic, ROC) 곡선 분석을 수행하였다. Firth의 편향-감소 로지스틱 회귀분석(Firth&#x02019;s bias-reduced logistic regression)을 이용하여 각 문항의 가중치를 산출하였다. 모델의 예측 능력은 곡선 아래 면적(area under curve, AUC)으로 평가하였으며, 민감도(sensitivity), 특이도 (specificity), 95% 신뢰구간(confidence interval, CI)을 함께 제시하였다.</p>
<p>본 연구의 통계 분석 및 그래프 생성에는 R(ver. 4.5.1, R Foundation for Statistical Computing, Vienna, Austria), logistf 패키지(cran.r-project.org), Prism 9(ver. 9.51, Graph-Pad Software, LLC, San Diego, USA), 그리고 SAS 9.4(SAS Institute Inc, Cary, USA)를 사용하였다. 모든 통계적 유의성 검정은 p&lt;0.05를 기준으로 하였다.</p>
</sec>
<sec sec-type="results">
<title>결과</title>
<p>전체 대상자의 평균 연령은 48.8 &#x000b1; 17.1 세, 여성이 58명으로 43.0%였다(<xref rid="t1-jkda-2026-64-6-001" ref-type="table">Table 1</xref>). 전체 대상자의 치주염 진단 기와 설문 응답 평균점수는 <xref rid="t2-jkda-2026-64-6-001" ref-type="table">Table 2</xref>에 기재하였다.</p>
<p>크론바흐 알파 계수를 이용한 신뢰도 분석에서 전체 18개 문항(PQ-18)의 크론바흐 알파(<xref rid="t3-jkda-2026-64-6-001" ref-type="table">Table 3</xref>)는 0.68이었고 신뢰도를 떨어뜨리는 15번 문항(Q15: 스케일링을 최근 2년간 한 적이 없다.)을 제외한 17문항의 크론바흐 알파는 0.69 (자료는 미제시)로 향상되었다.</p>
<p>주성분분석의 varimax 직교회전(orthogonal rotation)을 이용한 타당도 분석 결과에서는 PQ18에서 3개 성분까지 선택할 경우 총 분산의 36%를 설명할 수 있고, 그 중 Q15는 이들 세 성분에 포함되지 못하였다(<xref rid="t4-jkda-2026-64-6-001" ref-type="table">Table 4</xref>). 출혈이 있는 임플란트 Q3은 염증 증상의 Factor 1과 발거 경험 Factor 2 양측에 포함될 수 있었다. 한편 문항 Q15를 제외할 경우, 17개 문항 모두 3개 factor에 포함되며, 총분산의 37.9%까지 설명할 수 있었다(자료는 미제시). 따라서 17개 문항으로 구성된 PQ-17 설문지가 18개 문항의 PQ-18 설문지보다 높은 구성타당도(construct validity)를 가진다고 할 수 있다.</p>
<p>건강군을 치주염 1기이면서 탐침 시 출혈이 있는 치주낭 탐침 깊이가 4mm이상인 치아가 하나도 없을 때로 정하고, 질병 기준을 치주염 2기 이상 또는 치주염 1기 시 치주낭 깊이가 4mm 이상이면서 탐침 시 출혈이 있는 치아가 1개이상으로 규정하였을 때, 치주치료 필요가 보이지 않는 군은 13명, 필요한 군은 122명이었고, 각각의 설문지 점수는 <xref rid="t5-jkda-2026-64-6-001" ref-type="table">Table 5</xref>와 같다.</p>
<p>18문항합(PQ-18)의 수신자 조작 특성 그래프 분석 결과, 이 설문지의 AUC는 0.806 &#x000b1; 0.049(95% CI: 0.709-0.903, p&#x0003d;0.003)으로 높은 판별 성능을 보였다. 적합한 모형의 민감도는 0.762, 특이도는 0.769였다(<xref rid="f2-jkda-2026-64-6-001" ref-type="fig">Fig. 2</xref>). 17문항합(PQ-17)의 수신자 조작 특성 그래프 분석 결과 AUC는 0.821이고 민감도는 0.639, 특이도는 0.846이다(<xref rid="f3-jkda-2026-64-6-001" ref-type="fig">Fig. 3</xref>).</p>
<p>Q15를 제외한 PQ-17의 설문문항을 독립변수로 하는 Firth의 로지스틱 회귀분석을 수행하여 각 문항의 가중치를 산출하였다(<xref rid="t6-jkda-2026-64-6-001" ref-type="table">Table 6</xref>). 이 가중치를 활용하여 추정된 로지스틱 회귀식은 Logit(p&#x0005e;) &#x0003d;1.0648&#x02013; 0.0963Q1 &#x02013; 1.4862Q2 &#x0002b; 0.0741Q3 &#x0002b; 0.6092Q4 &#x02013; 0.9775Q5 &#x0002b; 1.5506Q6 &#x02013; 2.5641Q7 - 0.7188Q8 - 0.3784Q9 - 0.8742Q10 &#x0002b; 1.2415Q11 &#x0002b; 1.5902Q12 &#x0002b; 1.9711Q13 &#x0002b; 1.4141Q14 &#x0002b; 2.9699Q16 &#x02013; 0.3491Q17 &#x02013; 0.8334Q18이다. 이때 절편 1.0648을 제외한 나머지 각 문항의 계수(-0.0963, -1.4862. &#x02026;, -0.8334)가 17개 설문문항의 총점을 계산할 때 사용되는 가중치가 되었다.</p>
<p>Firth의 로지스틱 회귀분석을 통한 가중치 적용 시 AUC가 0.964로 크게 향상되어(약 18% 개선), 가중치 적용의 장점을 확인할 수 있었다. 이 가중치 모델에서 민감도는 0.893, 특이도는 1이었다(<xref rid="f4-jkda-2026-64-6-001" ref-type="fig">Fig. 4</xref>).</p>

<sec sec-type="discussion">
<title>고찰</title>
<p>크론바흐 알파 계수는 내적 일관성을 나타내는 지표로 알파 계수&#x0003d;문항수/(문항수-1)&#x0002a;(1-(각문항의 분산의 합)/(문항합의 분산))이다. 문항 수가 클수록 알파 계수가 높아지게 된다. 일반적으로 크론바흐의 알파계수가 0.7 이상이 되기를 목표로 하나, DeVellis와 Thorpe&#x0005b;<xref ref-type="bibr" rid="b14-jkda-2026-64-6-001">14</xref>&#x0005d;는 0.65-0.70 구간을 &#x00027;minimally acceptable&#x00027;, 0.70-0.80을 &#x00027;respectable&#x00027;로 분류했고. Ursachi 등&#x0005b;<xref ref-type="bibr" rid="b15-jkda-2026-64-6-001">15</xref>&#x0005d;, Taber&#x0005b;<xref ref-type="bibr" rid="b16-jkda-2026-64-6-001">16</xref>&#x0005d;, 그리고 van Griethuijsen 등&#x0005b;<xref ref-type="bibr" rid="b17-jkda-2026-64-6-001">17</xref>&#x0005d;의 연구에서는 0.6 이상도 수용 가능하다고 보고하였고, 다양한 요소가 영향을 미치는 다원성 질환인 치주질환&#x0005b;<xref ref-type="bibr" rid="b18-jkda-2026-64-6-001">18</xref>&#x0005d;의 설문 조사는 질환력, 가족력, 전신질환을 포함하는 다차원적인 설문으로 구성할 수밖에 없었기에, 18문항의 PQ-18이나 질문을 줄인 PQ-17의 모두의 신뢰도는 수용 가능한 것으로 판단된다.</p>
<p>주성분분석 결과 17개 문항으로 구성된 PQ-17 설문지가 18개 문항의 PQ-18 설문지보다 높은 구성타당도(construct validity)를 가진다고 할 수 있다. Q3은 출혈 있는 임플란트인데, 염증 증상으로 묶이는 Factor 1이나 발거 병력과 상관이 있는 Factor 2 양측 모두에 속할 수 있겠다. 부모의 치주질환 병력이 염증 증상과 같은 Factor로 치환되고, 당뇨병 유무가 치아 발거 병력과 가장 연관이 있으며, 흡연은 구조손실과 가장 관계가 있었다는 부분이 흥미로운 발견이다.</p>
<p>구성타당도 측면에서 주성분분석을 통해 추출된 3개 요인이 총 분산의 36.0%(Q15 제외 시 37.9%)를 설명한 결과는, 요인 분석에서 분산 설명력의 보편적 기준이 없다는 점을 고려할 때 문항 수 대비 설명력과 응답 부담 간 균형을 고려할 때 수용 가능한 수준으로 판단된다&#x0005b;<xref ref-type="bibr" rid="b19-jkda-2026-64-6-001">19</xref>&#x0005d;.</p>
<p>Q15(스케일링 미시행)이 전체 설문에 부정적 영향을 미친 원인으로, 응답군의 연령을 고려해야 한다. Q15 &#x00027;예&#x00027; 응답군은 평균 연령이 39.67 &#x000b1; 12.78세로 &#x00027;아니오&#x00027; 응답군 50.70 &#x000b1; 15.49세보다 유의하게 낮았고(p&#x0003d;0.0007), AVE.MBL도 13.12 &#x000b1; 7.65% vs 17.86 &#x000b1; 8.09%로 낮았다(p&#x0003d;0.0101). 이는 젊은 연령층이 스케일링을 받지 않았음에도 치주 상태가 양호한 패러독스를 반영하며, 예방행동과 연령 효과가 상충하는 구성무관 항목으로 해석된다.</p>
<p>PQ-18, PQ-17, 가중치부여 PQ17 모두 치주질환 상태 예측에 효과적이었다. 국내 (Jin 등&#x0005b;<xref ref-type="bibr" rid="b20-jkda-2026-64-6-001">20</xref>&#x0005d;), 미국 (Eke와 Dye&#x0005b;<xref ref-type="bibr" rid="b21-jkda-2026-64-6-001">21</xref>&#x0005d;, Eke 등,&#x0005b;<xref ref-type="bibr" rid="b22-jkda-2026-64-6-001">22</xref>&#x0005d;), 독일 (Renatus 등&#x0005b;<xref ref-type="bibr" rid="b23-jkda-2026-64-6-001">23</xref>&#x0005d;), 태국 (Lertpimonchai 등&#x0005b;<xref ref-type="bibr" rid="b24-jkda-2026-64-6-001">24</xref>&#x0005d;)에서 발표된 설문 도구를 이용한 치주질환 평가능을 본 연구에서 조사한 PQ 설문 문항들과 비교하면, 가중치를 부여한 17문항의 PQ17이 탁월한 성능을 보였고(<xref rid="f4-jkda-2026-64-6-001" ref-type="fig">Fig. 4</xref>) PQ-17 단순합의 경우도 AUC가 더 높았다(<xref rid="f3-jkda-2026-64-6-001" ref-type="fig">Fig. 3</xref>). Jin 등의 연구&#x0005b;<xref ref-type="bibr" rid="b20-jkda-2026-64-6-001">20</xref>&#x0005d;에서 사용된 문항은 14개로 구강상태에 대한 자가진단, 스케일링 경험 등을 포함한 14개 문항으로 AUC는 0.781, 특이도 0.77, 민감도 0.57이었다. 2009년 Eke 와 Dye의 연구&#x0005b;<xref ref-type="bibr" rid="b21-jkda-2026-64-6-001">21</xref>&#x0005d;는 AUC 0.7, 특이도 0.85, 민감도 0.48 이었고, 인구학적 특성을 추가한 2013년 Eke 등의 연구&#x0005b;<xref ref-type="bibr" rid="b22-jkda-2026-64-6-001">22</xref>&#x0005d;는 AUC는 0.81, 특이도 0.85, 민감도 0.58이었다. Renatus 등의 연구&#x0005b;<xref ref-type="bibr" rid="b23-jkda-2026-64-6-001">23</xref>&#x0005d;에서는 AUC가 0.81, 특이도 0.76, 민감도는 0.86이었다. Lertpimonchai 등의 연구&#x0005b;<xref ref-type="bibr" rid="b24-jkda-2026-64-6-001">24</xref>&#x0005d;에서는 AUC가 0.58, 특이도 0.90, 민감도 0.26이었다. 가중치 부여 PQ-17은 AUC 0.964, 특이도 1.0, 민감도 0.89(<xref rid="f4-jkda-2026-64-6-001" ref-type="fig">Fig. 4</xref>)로 탁월한 판별능을 보였다고 할 수 있으나, 현 연구에서는 건강군이 13명뿐이고, 이상적인 표본 수의 405명의 1/3에서 시행한 예비연구라는 점에서 과적합 가능성이 있다. 현 시점에서는 PQ-17 단순합(AUC 0.82, 민감도 0.64, 특이도 0.85)(<xref rid="f3-jkda-2026-64-6-001" ref-type="fig">Fig. 3</xref>)을 사용하여도 치주치료 필요 여부를 높은 수준에서 판별 가능하므로 이를 사용할 수도 있겠다. 그러나 문항별 가중치를 부여하였을 때 이 도구의 성능이 더욱 향상될 가능성을 확인할 수 있었다 .</p>
<p>본 연구의 한계는 실제 필요한 연구대상자 수 405명에 못 미치는 135명에서 임상검사와 설문조사를 시행하였다는 점에서 통계적 검정력이 부족할 수 있다는 점이 하나이고, 또한 서울특별시와 천안시에 근거를 두는 의료기관에서 연구대상자를 모집하였기에 타지역과 차이가 나타날 수 있다는 점이다. 아울러 피험자 모집광고를 내기는 하였으나, 이미 치과치료의 필요성을 인지하여 내원한 환자들이 많이 참여하였다는 점도 이 연구의 약점으로 후속연구에서 보완이 필요하다. 일반 인구집단을 대상으로 한 전향적 코호트 연구를 통해 실제 스크리닝 성능을 평가해야 하고, 2주 간격의 검사-재검사를 통한 시간적 안정성 검증이 필요하며, 성별 및 연령별 문항 기능 차이 평가가 앞으로의 과제이다.</p>
<p>본 연구에서 발표한 PQ 지수는 높은 민감도, 특이도, AUC를 달성하여, 치주질환의 진단 도구로 활용 가능하다.</p>
